Discharge planners, care managers, clinics and community partners can refer clients directly to our team. We accept referrals 24 hours a day, 7 days a week.
How it works
Send the referral form
Download and complete the RIS Recuperative Care Referral Form, then email it to angelica@risfoundation.org or frontdesk@risfoundation.org, or fax it to (213) 289-1371.
We confirm eligibility
We check plan enrollment and request authorization from the health plan.
Intake and transport
We schedule intake and coordinate the client's arrival, including transportation when needed.
Care starts on day one
The client moves in with 24/7 staff support, a care plan, in-house primary care and access to their medications.
